- Can you put any weight on it without sharp pain?
- Is the swelling around the ankle bone (sprain) or directly on the bone (possible fracture)?
- Did you hear or feel a 'pop' when it happened?

Sprain vs. fracture: what's the difference?
A sprain happens when the ligaments — the tough bands that hold your ankle together — get stretched or torn. A fracture means a broken bone. The two can feel surprisingly similar.
The classic clue: if you can't walk four steps without sharp, focused pain over a bone, or if the pain is right on the bone (not the soft tissue around it), you likely need an X-ray. Sprains usually hurt around the joint, with diffuse swelling.
What to do in the first 48 hours: RICE
Rest. Ice for 15–20 minutes every 2–3 hours. Compression with an elastic wrap. Elevation above heart level when possible.
Avoid heat, alcohol, and intense activity for the first 48 hours. Over-the-counter ibuprofen or acetaminophen can help with pain.
When most ankle sprains heal
Mild sprains usually feel much better in 5–7 days and are fully healed in 2–3 weeks. Moderate sprains can take 4–6 weeks. If you're still in pain after a week with no improvement, come in.
Come to urgent care for:
- Swelling, pain, or bruising you want X-rayed today
- Can't put much weight on it
- Pain that isn't improving after 48 hours
- You need a brace, splint, or crutches
Go to the ER (or call 911) for:
- Bone visibly out of place or skin broken over the injury
- Severe deformity or a foot/toes that look pale or blue
- Numbness or loss of feeling below the injury
- You fell from significant height or were in a major accident

Do I need an X-ray for every ankle injury?
Not always. Providers use the Ottawa Ankle Rules — if you can walk four steps and there's no pain on the bone itself, X-ray is usually not needed. We'll decide together.
How long until I can run again?
For mild sprains, usually 1–2 weeks of careful loading. Moderate sprains may need 4–6 weeks plus rehab. We'll give you a clear plan.
Should I keep walking on it?
Light weight-bearing as tolerated is generally fine after the first 48 hours. If walking causes sharp, focused pain — stop and come in.
