HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our pledge

EveryDay Healthcare is required by law to maintain the privacy of your protected health information ("PHI"), provide you with this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect. We take this responsibility seriously.

How we may use & disclose your PHI

We may use and share your health information for the following purposes:

  • Treatment. To provide, coordinate, or manage your healthcare — for example, sharing information with a specialist we refer you to, or with a hospital if you need further care.
  • Payment. To bill and obtain payment from you, your insurance, or another responsible party. This may include sharing information about treatment provided so your insurer pays our claim.
  • Healthcare operations. To run our clinic — quality assessment, training, accreditation, business management, and customer service.

Other permitted uses

We may also use or disclose your PHI without your written authorization in limited circumstances, including:

  • Appointment reminders and information about treatment alternatives or health-related benefits.
  • Public health activities (such as reporting communicable diseases).
  • Required reporting of abuse, neglect, or domestic violence.
  • Health oversight, judicial and administrative proceedings, and law enforcement when legally required.
  • To avert a serious threat to health or safety.
  • For workers' compensation as authorized by law.
  • To family or friends involved in your care, if you don't object.

Uses requiring your written authorization

Most uses and disclosures not described above will be made only with your written authorization. This includes most uses and disclosures of psychotherapy notes, uses for marketing, and any sale of your PHI. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.

Your rights

You have the following rights regarding your health information:

  • Access. Inspect and receive a copy of your medical record.
  • Amend. Request a correction to information you believe is incorrect or incomplete.
  • Accounting of disclosures. Request a list of certain disclosures we made of your PHI.
  • Request restrictions. Ask us to limit how we use or disclose your information. We will consider but are not always required to agree.
  • Confidential communications. Ask us to contact you in a specific way (for example, only at home or by mail).
  • Paper copy of this notice. Receive a paper copy on request, even if you agreed to receive it electronically.
  • Notification of breach. Be notified if there is a breach of unsecured PHI.

To exercise any of these rights, contact us using the information below. Some requests must be made in writing.

Our responsibilities

  • We are required by law to maintain the privacy and security of your PHI.
  • We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices described in this notice and give you a copy of it.
  • We will not use or share your information other than as described here unless you tell us we can in writing.

Changes to this notice

We may change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our offices, and on our website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us by calling (559) 225-4706 or by contacting us through our contact page. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights at hhs.gov/hipaa/filing-a-complaint. We will not retaliate against you for filing a complaint.
